The instructor shall notify the student of the finding of an offense and the penalty, copying Ombud@uky.edu for SACSCOC compliance, as soon as possible after the penalty has been determined.   Please also provide a copy of the evidence to the Academic Ombud for SACSCOC compliance.

For a template letter that includes the elements listed below, please email ombud@uky.edu and state whether it is the student's first, second, or third offense and the penalty to be imposed.

 

Notice is defined in SR 6.4.1.4 as:

"Notice" shall be sent to a student in writing by both regular mail and email to the student's addresses as they appear in the Registrar's records.  The University is not responsible for a student's failure to maintain current addresses in the Registrar's records.  Additionally, instructors are also encouraged to give notices to the student in person when feasible.

 

Any notice of a finding or penalty shall include the following:

  • student's name
  • ID number
  • college in which the student is enrolled
  • course and section in which the offense occurred
  • date and nature of the offense
  • penalty that is being imposed or recommended
  • any right that the student may have to appeal the finding or penalty
